Foster Parent Application

Apply to be a Foster or Adoptive Parent
Thank you for your interest in becoming a foster or adoptive parent with Foster in Texas. The application/verification process consists of three (3) phases:
- Phase 1: This is the initial application phase, and is required for both new and transfer families. Phase 1 consists of five (5) forms that must be completed by each household member. For your convenience, you can download the forms as a PDF from this page.


- Phase 2: After all Phase 1 forms have been completed, submitted, reviewed and approved; you will receive a username and password to log into FosTex, which is an online data management system. This is the first part of the verification phase, which must be completed within 30 days of receiving your FosTex username and password.

- Phase 3: This is the second and final part of the verification phase and must be completed within 45 days of receiving your FosTex username and password.
New Families
The first, most important, step of becoming a foster or adoptive parent is making the decision and commitment to help children in need. The next step is to begin the application process by completing and submitting all Phase 1 application forms. Thorough completion of the necessary paperwork will take some time and thought, but be assured that all the forms are important, so we know that the kids we place are in the safest, best quality homes. Transfer Families
If your home has already been verified for foster care in the state of Texas through another child placing agency and you wish to transfer and become part of our family, please also follow the Phase 1 instructions below.
Phase 1 — The Application
The process begins by downloading, completing, and mailing in the five (5) required forms. These forms have been merged into one PDF document and is available in English or Spanish. Certain forms are required to be completed by each household member over the age of 14 (see below). Along with your completed forms, you must include copies of driver’s licenses for household members age 16 and up, and copies of social security cards for household members 14 years and older.
- Download and save document
- Open and complete all forms. Type directly onto the document using your computer.
- Please make copies of the following sections and complete for every household member age 14 and over:
- Criminal History Disclosure
- Authorization for Criminal History and Central Registry Check
- Fair Credit Reporting Act Disclosure and Authorization
- Print the completed forms
- Sign all forms
- Make copies of driver’s licenses for household members age 16 and up, and copies of social security cards for household members age 14 years and older.
- Mail all original, signed forms, along with applicable copies of driver’s licenses and social security cards to:

Next Steps
When the Phase 1 application is received and reviewed, a criminal background, central registry, and driving record history check will be conducted. Once the criminal background, central registry, and driving record history checks are passed, you will receive a username and password to log into FosTex, our online data management system where you will proceed with forms for Phase 2 and Phase 3.
